If you visited Northbay Healthcare's website or patient portal between November 29, 2020, And May 14, 2024, then you may be a Class Member and eligible for benefits. Excluded from the Settlement Class are: (i) the officers and directors of Defendant and their affiliates, parents, and subsidiaries; (ii) any judge, justice, or judicial officer presiding over the Litigation and the members of their immediate families and judicial staff; (iii) any individual who timely and validly excludes themselves from the Settlement, and (iv) the successors or assigns of any such excluded persons.
If you are a Settlement Class Member and wish to receive a payment, you must complete a Claim Form online or postmarked on or before March 12, 2026.
Privacy Shield Pro Product: All Class Members are eligible for a one-year subscription to CyEx Privacy Shield Pro. This is a privacy protection service designed to enhance consumer data security and limit online tracking. All Class Members will receive an enrollment code by email or mail. If you are a Class Member but did not receive an enrollment code, please contact the Settlement Administrator.
Cash Payment: All Class Members may submit a claim for a one-time $15.00 cash payment.

To start your online claim, you must login with your Unique Login ID and PIN. You can find your ID and PIN on the Notice that was sent to you. The deadline to submit a Claim Form online or by email is March 12, 2026. If you are mailing your Claim Form, it must be mailed with a postmark date no later than March 12, 2026.
